Description:
Evergreen Healthcare is currently seeking a Registered Dietitian
for our Nevada Skilled nursing facilities. In this regional
position, the Dietitian is responsible for the regulatory compliance,
sanitation and infection control, meal preparation and service,
and nutritional planning for the five assigned facilities. The
Dietitian will visit assigned facilities to ensure compliance
with policy, law, and regulation; complete nutritional assessments
on all residents; make recommendations to clinicians regarding
nutritional care; monitor and evaluate residents identified

educational services regarding dietetics to staff and patients/residents;

Job Description:
In an acute rehabilitation hospital setting:
Plans and delivers nutritional care to patients according to
the physicians' diagnoses. Performs as a member of the multidisciplinary
care team. Screens newly admitted patients to identify those
at nutritional risk. Works in collaboration with Chef on dietary
needs of patients. Has the responsibility of daily management
of the food and dietitian service, implementing training programs
for dietary staff, and assuring that established policies and
procedures are maintained that address , safety practices for

Title: Registered Dietitian (Renal)
Location: Reno, NV
Job Description: The Renal Dietitian, in consultation
with the Attending Physician, is responsible for assessing the
nutritional and dietetic needs of each patient, recommending
therapeutic diets, counseling patients and their families on
prescribed diets, and monitoring adherence and response to diets.
Specific duties include:
- completing an initial Nutritional Assessment and developing
a Nutrition Care plan for each patient within 30 days of the
chronic in-center start date.
- performing ongoing nutritional monitoring with the review
of monthly lab results and subsequent nutritional changes followed
by recommendations to the patient and/or primary caregiver,
to improve patient outcomes.
- developing various education tools including food diaries,
bulletin boards, and seasonal posters to enhance patient and
staff education.
- maintaining nutritional outcomes and documentation required
by clinical policies and government guidelines (i.e. care plans,
risk management, CQI projects).
- attending and completing educational programs as required.
Requirements & Additional Information: Must hold
a baccalaureate or advanced degree with major studies in foot
and nutrition or dietetics. Must be registered or registry eligible
with the American Dietetic Association. Must have a minimum
of two years clinical experience, with at least one year working
with CKD/ESRD patients.

approx 10 hours per week. Duties include co-signing of the Tech
notes and RD documentation of high-risk residents/patients;
MDS assessments, care plans, RAPs. Monthly sanitation inspections
also performed by the RD. Long-term care experience helpful
but not necessary.
